*POSITION SUMMARY:*
The Home Health Aide is responsible for (1) providing various patient related services to meet the personal care needs and to promote comfort and independence to acutely ill and/or chronically ill patients in their homes; and (2) recording clinical and progress notes on Anchor's EHR system.
He/she works under the direct supervision of the designated supervisor and receives clinical direction from the designated Registered Nurse.
*POSITION QUALIFICATIONS:*
* Completion of state-approved Home Health Aide training program and/or successful passing of a written and field competency evaluation program which meets state and federal guidelines.
* Ability to relate to and care for acutely ill/chronically ill patients/families and their particular problems.
* Must be at least eighteen (18) years of age.
* Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
* Must possess a strong commitment to the goals, mission, and philosophy of the organization.
