Basic Purpose of Position: Assesses, plans, implements, and evaluated the nursing care of assigned residents.
Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs, and supportive staff members.
Participates in resident and family teaching.
Maintains role as resident advocate with a focus on Chase Nursing and Rehabilitation Center’s mission.
DUTIES AND RESPONSIBILITIES: Demonstrates Competency in the Following Areas: 1.
Assesses residents on admission and on an on-going basis.
Documents appropriately in the medical record.
2.
Assesses the resident’s condition and nursing needs; sets goals; prescribes appropriate nursing action.
3.
Documents changes in resident’s condition and nursing care on progress notes and plan of care.
4.
Submits written documentation in timely, clear, concise and pertinent manner.
5.
Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.
6.
Notes and carries out physician’s orders in a timely manner.
7.
Gives a thorough report to oncoming shift and documenting appropriately on calendar for upcoming needs.
8.
Follows through, as needed, on information given by shift report, resident of family regarding resident concerns.
9.
Follows through, as needed, on information given by shift report, resident or family regarding resident concerns.
10.
Implements plan of care of the resident based on assessments and goals as established by the interdisciplinary care team.
11.
Supervises other members of the healthcare team, as needed, to accomplish the plan of care which has been developed.
12.
Demonstrates competence in technical and manual skills, according to the scope of Nursing Practice Act for RN/LPN.
13.
Functions as a resident advocate by protection the resident’s rights.
14.
Administers medications using the six “rights” and performs treatments safely to assigned residents.
15.
Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
16.
Interacts with residents, families, co-workers, managers and other departmental personnel in a kind, knowledgeable and friendly manner.
17.
Consistently involves resident and family in planning for care, treatment, teaching needs and discharge plans.
18.
Updates plan of care, as appropriate.
19.
Functions as a leader for the CNAs and supportive staff by delegating in a manner that contributes to the excellence of the nursing unit to which the Staff Nurse in assigned.
20.
Participates in orientation of new personnel and performance evaluations of nursing staff.
21.
Follows proper infection control procedures and supervises supportive staff in infection control procedures.
22.
Demonstrates ability to remain calm in emergency and/or stressful situations and gives everyone a feeling of confidence that the situation is under control.
23.
Consistently follows the policies and procedures of Chase Nursing and Rehabilitation Center.
PROFESSIONAL REQUIREMENTS: Meets dress code standards; appearance is neat and clean Completes annual education requirements.
Maintains regulatory requirements.
Reports to work as scheduled.
Consistently completes and maintains assigned duties.
Wears identification while on duty; uses daily clock in system correctly.
Attends annual in-services as required.
Actively participates in the continuous quality improvement (CQI) activities.
Ensures confidentiality of patient’s records.
Consistently communicates the mission, ethics, and goals of Chase Nursing and Rehabilitation Center through actions and words.
Exhibits genuine interest in and appreciation of adults in later years of life.
Is a motivated, independent individual who can organize workload so that all functions are completed appropriately, in a timely manner.
REGUALTORY REQUIREMENTS: Current licensure as a Registered Nurse/Licensed Practical Nurse in the State of Indiana.
Current BCLS certification at the time of employment or within six (6) months of hire date.
Ability to read and communicate effectively in English.
