As a Health Center Pharmacy Manager, you will provide pharmaceutical services to patients to improve drug prescription usage and therapeutic outcomes.
This position will also collaborate with primary care providers and members of the care team on issues concerning medication therapy, toxicity of drugs and side effects as well as assisting in appropriate doses.
You will supervise the Staff Pharmacists and Pharmacy Technicians and report to the Director of Pharmacy.
JOB RESPONSIBILITIES:
Conducts and evaluates medication histories, assesses compliance and suggests modifications for optimal drug product selection in order to achieve desired outcomes; instructs patients in correct use of prescribed medications.
Individualizes medication regimens using sound principles and understanding of drug absorption, distribution, metabolism and elimination to establish and improve to optimum patient benefit.
Prevents, detects and resolves medication-related problem.
Makes evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team in a PCMH Model.
Effectively verifies patient medication orders for appropriateness and accuracy.
Provides medication reconciliation and evaluates medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information.
Works under collaborate drug therapy management agreements as applicable.
Leads pharmacy staff by assigning work tasks, supervising, training, evaluating, hiring, terminating, mentoring, and scheduling work shifts for all employees.
Precepts residents for medication distribution learning experience.
Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of drugs designated as hazardous waste.
Complies with all state and federal laws and regulations.
Prevents contraindication and performs duplicate therapy checks, in assessing and formulating recommendations to providers ensuring quality and economical drug therapy is performed daily.
Actively participates in the 340B drug pricing program including appropriate use of sliding scale for uninsured and underinsured patients.
Provides concise, applicable, and timely responses about medication queries from staff and patients.
Actively participates in development and implementation of related performance/quality improvement and patient safety activities.
Reports errors and adverse reactions promptly.
Provides information, education, and counseling to patients about medication-related care.
Identifies any barriers to patient compliance with drug therapy.
Participates in multidisciplinary reviews of patients progress.
Utilizes INSPECT to ensure appropriate use and dispensing of controlled substances.
Uses EHR appropriately and manages EHR tasks completely and efficiently.
Provides immunizations to patients when appropriate.
Communicates with payers to resolve issues that may impede access to medication therapies.

REQUIRED QUALIFICATIONS:
Education/Training
Bachelors Degree in Pharmacy
Doctor of Pharmacy (Pharm.
D.
) degree (preferred but not required).
Immunization certification
Experience
Five (5) years of pharmacy experience (preferred but not required)
Management experience (preferred but not required)
